Before calling to schedule your appointment, we require the following documentation from your physician: demographics sheet, script(s) {ORDERS not accepted} and the last clinic report.  Information may be faxed to 716-258-3329 OR 716-885-7317.

Appointment Policy: 

​

Effective 3/2/2020, we will be enforcing our NO SHOW or late RESCHEDULE policy.  All no-show appointment will be charged a $40 fee.  Rescheduling less than 24 hours from your appointment time may result in a $40 fee.

Insurance: 

​

We are committed to offering quality medical care to our patients with compassion and personalized attention.  We bill your insurance for your visit with us on your behalf.  However, patients and parents/guardians are responsible for obtaining required referrals and authorizations prior to their visit, if necessary.

​

Patient and parents/guardians are responsible for all copays, co-insurances and deductibles at the time of visit.  If you do NOT make this payment at the time of your visit, your apointment may be rescheduled.  A minimum of $250 is required for all co-insurance and deductible payments.
